Decisions

A decision you can explain years later.

Approve, approve with conditions, reject or defer. Each decision records who made it, why, under which policy, and on exactly which documents.

What every decision keeps

  • The named approver, their rationale and the review date.
  • The policy version and questionnaire version in force, and the exact evidence versions relied on.
  • A snapshot of the risks at the time, and any conditions with their due dates.

Rules that protect the decision

  • Decisions cannot be edited. An amendment is a new record linked to the original.
  • A critical finding blocks approval unless a person with authority to accept that risk records an exception.
  • Only people whose role includes approving can record a decision. Permissions are checked on every request.
  • Change the evidence, the questionnaire or the policy afterwards, and the original decision still shows what was known on the day.
